Phone Interview - Can you get an offer without a face-to-face?

Can you get a job offer based only on a phone interview? Are face-to-face interviews always done before every job offer?

I'm confused - what's the purpose of asking?  You can get an offer whenever the person wants to make one.  I've gotten an offer before without even having an interview.  It wasn't for a job I wanted, but that isn't the point.

I've hired some people only using Skype.  (We don't have funds to fly people in, and my candidate pool was limited to grad students in a particular program.)
It's turned out great each time, but I have good material to work with :)
